Case Management
ChildServe Case Management provides and promotes comprehensive, family-centered services and supports for children and young adults with disabilities living at home or in the community. ChildServe's case managers provide advocacy for consumers and are knowledgeable about many services and funding options for children and families.

    * In July 1998, ChildServe's case management program began by serving 110 children and young adults.
    * We now serve 270 individuals (242 children and 28 people 18 and over)
    * Any child served through ChildServe case management must live in their family home, relative or foster home, or a ChildServe Supported Community Living (SCL)-Residential group home and receive Home and Community Based Services (HCBS).
    * Case management is a Medicaid-funded service. 

Case managers:

    * are responsible for ensuring all approvals are in place for funding of services.
    * must have monthly contact on behalf of each person they serve.
    * have an average of 30 children on their caseload and visit an average of 15-20 children each month at their home, school or childcare.
    * must document each contact they have in progress notes.
